Property Damage

Where damage to University property has occurred, follow the instructions below:

  1. Report incident to your supervisor and the Chair or Director of the affected department.
  2. Complete and forward a Faculty/Department Incident & Investigation Report to EHS via fax (780-492-7790) within 48 hours of discovery of the damage.
